PATHWays Transitional Housing - PATH West Los Angeles

Individuals go through the following components to complete the program and successfully obtain housing and employment:

Assessment – During the assessment process we gather client information through questioning, listening, observation and other sources. Based on this information, we develop a description of the client’s status including her/his strengths, needs, resources, and challenges. Effective assessment provides a solid foundation for the work we provide clients as we help them identify goals and create strategies for achieving them.
Individual Service Plan (ISP)- Each client is given an Individual Service Plan (ISP), that structures the setting of goals, steps and the timelines that will enable the client to achieve independent living and self sufficiency.
Case Management- Upon enrollment, each client establishes an ongoing relationship with a case manager to promote dialogue, accountability and regular contact throughout their stay. Case management sessions include a series of coordinated activities with the client to track the progress of the ISP. Additionally, during the case management process, PATH will help identify clients’ underlying issues and needs, facilitate access to needed services, help achieve stabilization and secure affordable, permanent housing.
Life skills Workshops- PATH offers life skills workshops to promote opportunities toward greater personal responsibility, self-sufficiency and independence. Currently, the following workshops are scheduled at the PATH Westside Center:
