A Model for the Region

Intake for housing at PATH San Diego's Connections Housing is done through PATH’s collaborative integrated street outreach efforts.

Housing Resources

PATH San Diego's primary goal is helping homeless people off the streets and into permanent housing.

Interim Housing: 

Interim Housing provides a short-term stay, up to 90 days, with the goal of moving individuals who are homeless, chronically homeless, and deemed vulnerable or at risk on the streets into permanent housing.

Our Interim Housing program accommodates 150 people at one time, and is designed to move people off the street quickly. Clients using this short-term housing are encouraged to stay in the building throughout the day to access the three dozen plus services provided in the PATH Depot; everything from mental health to creative writing, from music classes and legal-aid, to resume-building workshops- virtually every need a person experiencing homelessness might encounter.

Permanent Supportive Housing: 

Permanent supportive housing provides eligible tenants with an affordable studio apartment unit coupled with supportive services designed to address the causes of an individuals previous homelessness. PATH San Diego's Connections Housing has 73 permanent studio units, and each resident is assigned to a case manager who assists them in accessing on-site services based on their needs. The apartments are subsidized with housing vouchers from the San Diego Housing Commission, with case management provided by PATH.

Priority is given to individuals who are chronically homeless and disabled in downtown San Diego; those who are considered most vulnerable on the streets, and who are frequent users of public systems such as emergency rooms.


Outreach and Supportive Services

Downtown Street Outreach and Individual Assessments: 

Outreach workers individually interview people experiencing homelessness in the neighborhoods surrounding Connections Housing to identify how best to help each person move from the streets to housing.

Employment Services:

Employment specialists help individuals return to the workforce. Veterans specifically meet with specialists through the Homeless Veteran Reintegration Program (HVRP). Participants are provided with concrete job skills, career counseling, vocational training, job placement assistance, and retention services. We focus our services on preparing participants for employment in industries that pay living wages and show high potential for individual growth. 

The PATH Depot: 

An on-site "One Stop Service Center" with approximately 30 nonprofit and community partners provides supportive services to address the root causes of homelessness. The PATH Depot provides services for those living in permanent and interim housing on-site in addition to PATH San Diego alumni, and other individuals participating in specialized programs that take place through the PATH Depot. Family Health Centers of San Diego provides caring, affordable, high quality healthcare and supportive services to everyone, with a special commitment to uninsured, low income and medically underserved persons.

Health Care

The Family Health Center at PATH San Diego's Connections Housing offers comprehensive preventive and primary care, as well as mental health care. The health center's hours are Monday through Friday from 8:00 a.m. to 5:00 p.m. and closed for lunch 12:00pm - 1:00pm. The clinic is open to the entire community.
