PATH provides the support that homeless individuals and families need in order to successfully transition from living on the street to thriving in homes of their own. High-quality supportive services are critical to ensuring that the people we serve are able to not only move into permanent homes, but stay in permanent homes long-term. The services we provide are available to anyone who is homeless or at risk of becoming homeless.

How Do I Get Started?
If you are homeless and are residing in the following communities, please contact our street outreach team through the phone numbers listed below:
Our outreach teams are typically out providing services between 7:30 a.m. and 4:00 p.m., Monday to Friday.
Because our teams are community based, the above listed numbers are for voicemails only. To help us reach you as quick as possible, please be sure to include the following information in your message:

After hours service requests should be directed to the LA County 211 hotline. Dial 211 directly on your phone for assistance.
If you are homeless or at risk of becoming homeless and are NOT residing in the communities listed under street outreach services, please contact us via phone or visit us at the PATH Mall for basic referrals and to schedule an official intake for services.
