Outreach and Referrals

Strategic Outreach Teams: Outreach workers visit six targeted areas in Los Angeles. Case managers travel in vans and on foot to provide homeless individuals with food and general hygiene kits. The outreach teams also encourage homeless people to utilize PATH services and resources.
PATH Access Center: The Center provides initial intake and assessment of new and current clients. The Center serves as the main hub for PATH internal and external referrals to programs and agencies.
