Provide case management to participants of a homeless social service center. Responsible for providing assistance and linkages for homeless adults and families. The Case Manager will conduct initial assessments, develop a case plan, and provide crisis intervention and resource referrals. Responsible for maintaining accurate records and documentation of achievements leading to stable housing. May require some evening hours. All positions require the following: Good communication skills; proficiency in Microsoft Office. Salaries commensurate with experience. Full-time positions include full benefits package covering medical, dental, vision, and pension. Fingerprint clearance and background check required.
