Provide case management to participants of a homeless social service center. Responsible for providing assistance and linkages for homeless adults and families. The Case Manager will conduct initial assessments, develop a case plan, and provide crisis intervention and resource referrals. Responsible for maintaining accurate records and documentation of achievements leading to stable housing. May require some evening hours.

The Data Specialist for the Homeless Prevention and Rapid Re-Housing Programs (HPRP), is responsible for data input, analysis and evaluation of HPRP client tracking data. The Data Specialist will provide data entry and review data entered by partner agencies, compare data input in HMIS with client files for quality control and data accuracy, and monitor the data for progress towards established program goals. The Data Specialist will collect data from partners, prepare reports and document quality control outcomes with recommendations for corrective actions. Previous experience with Homeless Management Information System (HMIS) is preferred.

The Program Manager is responsible for the daily operations of PATHWays Residential Housing, a 98-bed facility at The Regional Homeless Center. Manages the day-to-day operations of the program including staff and facility oversight. Responsible for staff supervision, scheduling, contract reporting/compliance, client relations, resident workshops, and volunteer management. Responsible for day to day management of PATHWays program. This includes internal monthly reports, grant reports, workshops, staffing, client relations, case management, intake/assessment, volunteers, agency outreach, follow-up, and other activities related to operations. Program enhancement and development coordinated with other program management staff. Ensures quality service delivery to clients and positive client relations. Supervises Residential Associate staff, Intake Specialist, and Case Managers. Responsible for scheduling and job performance of line staff. Works with the Area Director and personnel during the hiring and disciplinary process. Coordinates relevant training sessions for staff development. Networks with local agencies and coalitions to increase resources available to program participants. Outreach to other homeless service providers regarding program services and availability. In cooperation with senior and program management, solicit local corporations for program support and attend meetings and coalitions for community building purposes. In addition to in-house program audits, perform random program evaluations to ensure compliance and quality service delivery. Develops, distributes, and collects client surveys. Develops client progress evaluations tools. Manages housing program budgets and spending. Monitors compliance with contract scope of services. Prepares monthly, quarterly, and annual reports for Hollywood contracts. Maintains the accuracy of information reported or recorded in program files and databases.
