The Housing Locator assists homeless veterans, homeless individuals and families in identifying and transitioning into a full range of permanent housing opportunities, including subsidized housing such as Section 8, Shelter Plus Care, and VASH. The Housing Locator will work with the navigators to support the housing needs of participants in the PATH programs.

PATH - Los Angeles

Provide case management to the participants of the Supportive Services for Veteran Families (SSVF). Responsible for conducting  initial  assessments,  develop  individual  service  plans,  provide  crisis  intervention,  provide  supportive service referrals, and maintain accurate records and documentation of progress notes. The case manager will assess and  determine  appropriate  levels  of  financial  assistance  to  the  participants,  conduct  in-home  visits,  and  track  and monitor  household  usage  of  financial  assistance. The case manager is required to provide documentation and data management through HMIS.

All positions require the following: Good communication skills; proficiency in Microsoft Office. Salaries commensurate with experience. Full-time positions include benefits package covering medical, dental, vision, and pension. Fingerprint clearance and background check required.
